Are you a CHW? Consider taking this survey to share your perspective about organizational readiness to change and interprofessional collaboration and be entered to win a $100 gift card! Background information: The purpose of this study is to investigate the relationship between organizational readiness to change and interprofessional team collaboration. I anticipate 130 to 200 CHWs will participate in the study. The term interprofessional refers to a group of individuals from different disciplines who work and communicate with each other (Hall & Weaver, 2001). This includes physicians, nurses, social workers, dietitians, community health workers and others. Collaboration is the “joint communicating and decision-making process with the expressed goal of satisfying the patient’s wellness and illness needs while respecting the unique qualities and abilities of each professional (Coluccio & Maguire, 1983, p59). Procedures: If you choose to participate in this study, you will answer questions related to your perceptions of interprofessional team collaboration and organizational readiness to change. The questionnaire is anonymous and will take less than 10 minutes to complete. You can refuse to answer any questions and can withdraw from the study at any time without penalty. Upon completion of the survey you will have the opportunity to register for an incentive. The researcher and faculty advisers will have access to the data. The results of this research study will be published, but your name and contact information will not be used in any form. I seek to maintain the confidentiality of all data and records associated with your participation in this research. Further, any communication via the Internet poses minimal risk of a breach of confidentiality. Data will be stored on a password protected computer. The raffle registration file will be deleted once the survey is closed and a raffle winner is identified using a random number generator. The winner will be contacted via email. Risks and benefits of being in the study: There are no foreseeable risks or discomforts if you agree to participate in this study. Although there may be no direct benefit to you, the possible benefit of your participation is the furthering of research involving community health workers as part of the practice team. Compensation: You will receive no compensation for participating in this study.
