Nevada Health Centers is looking to hire two Navigators! The Navigator will assist NVHC patients by providing support to navigate patients through the health and social services in an effort to overcome barriers to care and decrease health disparities. The navigator is responsible for identifying patients served by NVHC, and monitoring selected patients to ensure compliance with health screenings and treatment plans. This may include, but is not limited to encouraging appointments for screenings, diagnostic testing and follow-up exams for the detection and treatment of cervical, breast and colon cancer, as well as scheduling follow-up exams for the prevention, management and treatment of chronic diseases. The Navigator will participate in training programs to learn the best- practice methods for effective patient navigation and/or education.
